Funding Application
Terms & Conditions for Funding by Noah’s Smile
Purpose of Funding:
Funds provided by Noah’s Smile, Inc. must be used solely for the stated purpose as outlined in the application (e.g., travel, lodging, medical expenses). Any changes to the intended use of funds must receive prior written approval from Noah’s Smile, Inc.
Use of Funds:
Recipients agree to use funds only for expenses directly related to the child’s medical care or necessary family support associated with treatment. Funds are strictly prohibited from being used for unrelated or non-essential expenses, such as personal entertainment.
Documentation and Receipts:
Recipients may be asked to submit receipts or other documentation to verify that funds were used according to the stated purpose. Required documentation should be submitted to Noah’s Smile, Inc. within 30-60 days after receiving funds or completing relevant travel.
Refund Policy:
Any funds that are unused or not applied to the specified purpose must be returned to Noah’s Smile, Inc. In cases of misuse or unaccounted-for funds, recipients may be asked to reimburse the charity.
Reporting Requirements:
Noah’s Smile, Inc. may request a follow-up report on the funded expenses, particularly if the support was for ongoing treatment or significant medical costs. Recipients may also be invited to share updates on the child’s health or provide a testimonial to help raise awareness, though this is optional.
Confidentiality and Data Usage:
Noah’s Smile, Inc. will handle all personal information with confidentiality, using it only as agreed upon in the consent form. By accepting funding, recipients agree to allow Noah’s Smile, Inc. to use anonymized information for reporting, marketing, or fundraising to support the charity’s mission.
Non-Transferability:
Funding from Noah’s Smile, Inc. is awarded specifically to the child and family named in the application. It cannot be transferred or used for other individuals or purposes.
Termination and Revocation of Funds:
Noah’s Smile, Inc. reserves the right to terminate funding if any terms and conditions are violated. If any information provided is found to be false or misleading, funding may be revoked, and any remaining funds must be returned.
Legal Compliance:
Recipients agree to comply with all relevant laws and regulations concerning the use of funds. Noah’s Smile, Inc. will not be held liable for any legal issues arising from misuse of funds by recipients.
Agreement to Terms:
By accepting funding from Noah’s Smile, Inc., recipients agree to abide by these terms and conditions. Recipients may be required to sign a written acknowledgment or agreement as part of the application or funding process.
